Call Safe Choices Senior Care Planning if your patient is experiencing:
- increasing physical disability,
- cognitive impairment, and/or
- inability to safely manage activities of daily living.
We accept calls and referrals from:
- Family members
- Physicians
- Hospitals
- Insurance companies
Fees for services are generally covered by Medicare and major insurance plans with a physician's approval. Private pay for services is also available.
