Specialty Programs
CVHC developed these special programs to meet patient needs.
- Heart Failure Management: Eliminate or greatly reduce hospitalizations and emergency room visits for the HF patient. Treatment and follow-up is provided in the home by experienced ICU/CCU RNs. Nursing care can range from teaching self-management to administering infusions. Telemonitors are used to track vital signs and weight every day. Most CVHC patients learn to manage their HF and stay out of the hospital with teaching from their home care nurse
- Anticoagulation Program: Reduces risks and increases positive outcomes for people taking blood-thinner medications. Blood-thinners such as warfarin are closely monitored and regulated. RNs perform PT/INR testing and get immediate results in the home. Physicians receive test results within minutes of testing. Teaching is provided to patients and families on all the important aspects of safe and effective treatment with blood-thinners.
- Home DVT Treatment: Our specialized home care program enables patients to receive heparin injections and warfarin monitoring for uncomplicated DVT (deep vein thrombosis or blood clot) in the home setting . Patients can save 1-5 hospital days, or may receive all of their treatment at home, avoiding hospitalization completely.
- Specialized Drug Therapies: Programs for providing continuous or intermittent infusions as ordered by physician to treat or palliate symptoms of heart failure that are not controlled by diuretics and oral medications alone. Home monitoring and titration; side-effect monitoring and intervention; and EKG/patient response reported to physician are provided. Patients who have safely received these infusions in the hospital are candidates to receive them at home.
- Diagnostics: Remote telemonitoring of vital signs and weight, onsite EKG, pulse oximetry, chest x-ray, and INR testing.
