Benefits
of Specialized Home Care
With specialized
home care, heart patients can achieve and maintain their optimum
level of health. Our approach is so effective that it reduces
or eliminates emergency room visits and hospital stays.

Keys
to Successful Home Management
- Cardiac
Care RNs who have made a transition to home care.
Each CVHC nurse has at least five years of experience in cardiac
care and is knowledgeable in care and treatment of heart, lung,
and related health problems.
- Advanced
treatments. Treatments, which a few years ago were available
only in the hospital, are now being effectively administered
in the home. Our nurses work with your doctor to provide these
treatments at home, often preventing hospitalization.
- Comprehensive
teaching. Our nurses will teach you and your family about
your treatment and medications, enhancing your ability to manage
your health independently. We're here to support you 24-hours
a day.
- Coordination
of services. Cardiovascular Home Care also provides therapists,
social workers, and home health aides to complete your circle
of care.
