Welcome to CardioVascular Home Care, Inc., an RN-founded, owned and managed home healthcare company providing a high level of technology-supported, consistent, quality nursing and ancillary services to patients with chronic heart and lung conditions in the Greater Fort Worth metropolitan area since 1994.
In 1998, we added a rehabilitation service for orthopedic patients and physicians called Orthopedic Home Care, to begin the smooth, successful recovery from joint replacement surgery at home.
Our "telehealth" program began in 2005. Seniors now can have their pulse, blood pressure, oxygen level, and weight monitored daily by an RN via a monitor in their home that transmits to CVHC every day. If vital signs aren't normal or if we don't hear from the senior, one of our RNs will call and/or visit right away.
In 2006, we developed Safe Choices, a senior advocacy/geriatric care management program that provides worried families and concerned physicians with an evaluation of the changing needs of seniors who begin to have problems with daily life and new services as needed. This service is available directly to seniors and families without a doctor's order. Family members who do not live close to their seniors can arrange for Safe Choices by phone.
A professional advisory board, led by a cardiologist medical director, guides the delivery of the agency's services.
